- The registration forms contain
your personal information, billing information, how you want
us to handle any prescriptions, your medical
history, and a consent and release.
- You will be prompted to select your consulting physician.
- The initial consultation fee for the in person examination is $150.00. Follow up consults are $155.00.

| | 2. | The second step is to establish records with our office. Please fax to 866-766-4056.

A copy of your Identification (Drivers License, Passport, or valid Military Identification.

Any existing medical records, findings, or test results pertinent to your
medical complaint. If you do not have medical records, records will be
established with our office during your initial in person visit with your
consulting physician.
